Submit a request to change information in a student's program enrollment record. Types of information that may be changed:
- Major 1
- Major 2
- Major Catalog
- Concentration 1
- Concentration 2
- Minor 1
- Minor 2
- Minor Catalog
You must use the Major Declaration Selection Tool that is located in the MyColumbia Portal - Advising Tab.
You will copy and paste the results from the tool into the form.
